At my first job as the IT officer at a local NGO in Rwanda, I watched field workers write leave requests by hand and mail them to headquarters just to get a stamp and have them sent back, all for a single day off.
But leave requests were just the tip of the iceberg: payments sat in piles waiting for physical signatures, asset records lived in cabinets nobody wanted to dig through, and important documents just vanished without a trace.
